Understanding Clinical Profile: Clinical Care Needs, Programs & Type of Care

Clinical Profile is a new section on the referral and resident record with three structured fields describing a resident's clinical situation for their current stay: Clinical care needs, Programs, and Type of care. Instead of typing conditions into a task or comment, your team tags them directly on the record, then filters by those values to find matching referrals or residents.

Who this is for: Admissions Directors, Clinical Liaisons, and other Screener users at SNF and LTACH facilities. Other facility types will only see Clinical care needs; Programs and Type of care are SNF-only in this release.

Before you start

  • No special permission is needed. Anyone who can already edit referral details can add or change these fields.

  • The same three fields apply to both referrals (pre-move-in) and residents (post-move-in); they're the same underlying data.

How to add or edit Clinical Profile values

  1. Open the referral or resident record you want to tag.

  2. Find the Clinical Profile section, either in the referral header or in the Referral Information section of the Facesheet tab.

  3. Select Clinical care needs, Programs, or Type of care to open its picker. Only one section opens at a time; opening Programs collapses Type of care, and vice versa.

  4. Search or scroll to find the value you need. All three lists are sorted alphabetically.

  5. Select a value to apply it. Clinical care needs and Programs allow multiple selections; Type of care allows only one.

  6. Remove a value anytime by selecting the (X) on its chip.

You can also skip opening the record entirely. From the screener grid or the Residents page, select the three-dot menu → Edit referral details on any row.

Expected result Once applied, a value shows as a chip in the referral header, on the Facesheet, and, for SNF/LTACH residents, as a column on the Residents page. From there you can:

  • Filter the screener or Residents page by any of the three fields. Multiple values within one field is an OR match (Trach or Vent); combining fields is an AND match (Trach and Subacute Rehab).

  • Use the "No tag" filter option to find referrals or residents that haven't been tagged yet.

  • On the Residents page, use Customize Table (max 9 visible columns) to show or hide Type of care, Programs, Clinical care needs, and Resident tags. Your choice is saved per user, across all facilities.


Common issues and fixes

Issue: I don't see Programs or Type of care on a referral or resident.

Fix: Those two fields only apply to SNF facilities. Home Health, Hospice, Assisted Living, and Independent Living facilities will only see Clinical care needs.

Issue: I can't find these fields as columns in the screener grid.

Fix: Right now the screener supports filtering by these fields only; they're not shown as row values yet. They're already available as columns on the Residents page.

Issue: The value I need isn't in any of the three lists.

Fix: All three lists are managed by exacare centrally, not per facility. Contact exacare Support with the value you need, and we'll get it added to the master list for everyone.

When to contact exacare Support Contact exacare Support if a value you need is missing from any of the three lists, a facility that should have Programs or Type of care (SNF or LTACH) isn't showing them, or a value you apply doesn't save or disappears after you apply it.
